I expect that most Practices will ask their Practice Manager to set up a contract between us. I will contact and correspond with whoever approaches me, with the assumption that they represent the Practice, unless advised otherwise.

1. Please read the section entitled Terms & Conditions .

2. Then contact me via email or phone to see whether I am available for the dates proposed. Please include any relevant details about the service you require. I will reply promptly, by e-mail or phone.

3. If I am available for the dates requested I will then telephone or email the Practice to secure the details of the service requested, and to agree the Fees for this.

4. And I will then e-mail the Practice, with a document attached that details the booking, to allow the Practice to cross-check their record of the arrangment between us.

5. The Practice then need to e-mail me a reply confirming the booking and the details of our agreement. I will accept this e-mail reply as confirmation of the booking. 

6. When I arrive at a Practice in which I have not worked before I'll bring with me a 'personal pack' containing a set of good scan copies (not the originals) of all the relevant documents that confirm my identity, my eligibility to work as a GP and that I am properly indemnified. I do have an NHS smart card.

7. It is my practice to attend my first session 30-mins early (not charged) in order to arrange passwords and logins, to familiarise myself with the practice premises and to supply documents for checking by the Practice manager.
